Purpose

To fulfill ºÚÁϹÙÍø transcript requests.

Policy

Students may visit www.minnesota.edu/forms to request a transcript. There is a fee charged for this service.

Responsibility

Vice President of Academic and Student Affairs

Review policy on an annual basis.

Registrar or designee

Process transcript requests.

Student Services Personnel

Inform ºÚÁϹÙÍøs of policy and assist ºÚÁϹÙÍøs with making requests.
